Where do I park?
Please see the maps and directions page.

Is health insurance accepted?
We accept Medicare, Medicaid, and most private health insurance plans, including the following:

  • Blue Cross/Blue Shield
  • Presbyterian
  • United Healthcare

What do I bring to my first visit?
First-time patients should arrive 10-15 minutes before the appointment time to complete paperwork and bring the following items:

  • Insurance card and personal identifications
  • List of questions for the clinicians
  • List of medications

What can I expect at my first visit?
When a patient arrives at the clinic, we will take a copy of his/her personal ID and insurance cards. Each patient will complete a patient questionnaire. The initial appointment takes approximately 2 hours.

Based on the outcome of the assessments, the patient may be scheduled for a follow-up appointment in the clinic or we will help identify the appropriate providers in his/her community. Family members, significant others, or friends are welcome. We ask that patients make child care arrangements for small children so that the evaluations can proceed in a timely fashion.
